Post Hospitalization Care

Hospitalization seems like an inevitable part of the ageing process. However, Medicare data suggests that nearly 20 percent of patients are re-hospitalized within 30 days of being discharged. This high rate of readmission is usually because of the patient ignoring the doctor’s recommendations, not following the medical instructions strictly, or not having the right environment to promote proper and quick recovery.

Proper care and assistance in the first few weeks after discharge can help reduce the likelihood of readmittance. Our caregivers at Irish Help at Home have been providing assistance and home care to our clients post-hospitalization since 1996. We offer comprehensive follow up care at the homes of our clients and help them on their path to recovery.

What to Expect with Post-Hospitalization Care

After hospitalization, you or your loved one will need help performing some tasks that you cannot handle yourself. Sometimes, basic chores like bathing and dressing can prove to be challenging, not to mention changing bandages on a wound or setting up the intravenous (IV) system to administer drugs.

Your doctor may also recommend special equipment like a bath seat, a walker, or commode to make some tasks easier and reduce the risk of injury during recovery.
